Are you licensed, bonded and insured?
Yes, we hold state licenses, bonding and insurance. Our bonding and employee theft policies are in excess of ten million dollars. If requested a Certificate of Insurance can be issued in your name, as an additional named insured. Our broker, Aon Risk Services, can be contacted to verify that we have never had a claim against us.
If my material is stored in hard to access locations can you provide the necessary labor to remove the material? For example, if is it located in a basement, attic, or off site storage facility can you still accommodate us?
We can supply all necessary labor and equipment to remove your material wherever you may have it located. Recently we had a request to destroy the documents of a local well known dairy farm. The files and records were stored in a barn loft that was occupied by milk cows. We accomplished this task with no problem and I am sure that regardless of where your material is stored we will accomplish it's retrieval without incident.
If I chose to have in-house Document Destruction can I personally witness the Destruction Process?
Yes, Accurate's Witness Protection Program provides a viewing booth which can accommodate up to four persons with closed circuit TV for monitoring the shredding of your documents.
What happens to my material after it is shredded?
All shredded material is baled and recycled. It is shipped to paper mills where it is used as a pulp substitutes sparing the harvesting of new trees for their pulp content. When you receive our invoice you also receive a recycling report on the grades of paper recycled and it's weight.
Are you approved to do HIPAA related document destruction of medical records and medical electronically stored records?
Accurate has been approved as a Business Associate, by the State of New Jersey for the destruction by shredding, of all medical records. Electronically stored medical records follow the HIPAA guidelines for magnetic destruction of electronically stored information. NOTE: Whom ever you use for your destruction of HIPAA regulated material; you must enter into a Business Associate Agreement with them. If your present vendor does not know what your talking about when you ask for it, call us we will be happy to e-mail you a Business Associate Agreement Form.
Under the HIPAA regulations, is it advisable that medical records should be destroyed?
If a covered entity fails to apply destruction policies uniformly or where destruction is contrary to policy, courts may allow a jury to infer in a negligence suit that if records were available, they would show the facility acted improperly in treating the patient.
Should I enter a contract with the company I choose to do Document Destruction?
Federal Requirements Under the HIPAA privacy rule. If a destruction services is contracted to do document destruction, the contract must provide that the Business Associate will establish the permitted and required uses and disclosures of information. The contract may not authorize the business associate to use or further disclose protected health information in a manner that would violate the requirements if done by the covered entity. The contract must authorize the covered entity to terminate the contract if the covered entity determines that the business associate has violated a material term of the contract.
Are there recommended ways to achieve document destruction?
AHIMA recommends the following: Destroy the records so there is no possibility of reconstruction. Appropriate methods for destroying paper records include burning, shredding, and pulverizing. The laser disks used in write once-read many (WORM) document imaging applications cannot be altered or reused, making pulverization an appropriate means of destruction. The preferred method for destroying computerized data is magnetic degaussing. (Data are stored in magnetic media by making very small areas called magnetic domains change their magnetic alignment to be in the direction of an applied magnetic field. Degaussing leaves the domains in random patterns with no preference to orientation, rendering previous data unrecoverable.) NOTE: ACCURATE HAS USED MAGNETIC DEGAUSSING FOR THE PAST TWO YEARS TO DESTROY COMPUTERIZED DATA.
